The following students will need to submit a Physical Exam Form (note that this is a 2-page form) by Monday, August 1, 2011, documenting a physical exam given on or after September 1, 2010:

• All students upon intial enrollment to Swain
• All students entering Kindergarten
• All students entering grade 6
• All students in grades 5-8 who participate in school-sponsored after school sports
• Physical Exam Form (note this is a 2-page form) - return by Monday, August 1, 2011

ALL STUDENTS will be asked to submit an Annual Health Update (completed by the parent) by Monday, August 1, 2011.

DENTAL exams must be completed for students entering Kindergarten, 3rd and 7th grades. Click here for a copy of the form, which is due Monday, August 1, 2011.

MEDICATION PERMISSION FORMS

• Physician Order for Individual Student Medications for grades Preschool - 8,  if applicable; due Monday, August 1, 2011, or submit as needed
• Health Room Treatment Authorization Form for all students; due Monday, August 1, 2011

ATHLETES (grades 5-8):

Athletes must also submit a Physical Form, as referenced above, no matter what grade they are in.

• Athletic Physician's Statement - due Monday, August 1, 2011
• Athletic Parent's Permission Form - due Monday, August 1, 2011
